EVE-QAS Registration form

The ESVE and Nation Wide Specialist Laboratories maintain a confidentiality agreement to ensure that knowledge of your participation in the scheme will be used only for the purposes of administering the scheme.

You will submit your EQA results online directly to the scheme coordinator and they will not be seen by NationWide Specialist Laboratories. The scheme coordinator is currently Peter Graham. You may be contacted by a member of the ESVE quality committee or the scheme coordinator to discuss your results. Your results will not be disclosed to any other party.

If the contact person is not an ESVE/SCE member who is the nominate ESVE/SCE member for the laboratory? A copy of your EQA results will be sent to this individual.
